    Community Memorial Health System

    (Ventura, California) In honor of National Healthcare Decisions Day, Community Memorial Health System is encouraging community members to create their advance directives while enjoying a virtual night at the art gallery.

    This free online event takes place from 6-7 p.m. on Wednesday, April 21. To RSVP, visit www.cmhshealth.org/RSVP.

    Advance directives are documents that tell others how you want medical decisions to be made for your care if you can’t speak for yourself. They allow you to give your instructions in cases when you may be unconscious or impaired by dementia or other health conditions. These legal documents tell your family, friends and healthcare providers what kind of care you want, what you don’t want, and who you’d like to make healthcare decisions for you if needed. 

    Community Memorial Health System will support people through the process as they join CMHS for a special event enjoying a virtual night at the art gallery featuring music and the works of gifted artists. All the while, participants can complete an advance directive from the comfort of their own home.

    Community Memorial Health System is a not-for-profit health system, which is comprised of Community Memorial Hospital, Ojai Valley Community Hospital, the Centers for Family Health, and various outpatient centers serving communities located within Ventura County, California.
